Andover Sport Awards 2012
Once again, the team at Valley Leisure are hosting the Andover Sport Awards. The date of the event is Friday 14th December at the Andover Leisure Centre. It's an evening of celebration for all things sport in North West Hampshire.
To nominate for personal achievement in sport, significant contribution to sport or success in sport, choose the category below and you can enter online or print the form and send to the address below. Please note you must have a current photo of the nominee.
All nominations will go forward for inclusion in the Sport Hampshire and Isle of Wight Annual Sports Awards.

The new closing date for entries is midnight on Sunday October 28th 2012.
